There are a few ways to apply for coverage:

Use Kaiser Permanente's online broker quoting tool
  • Using the quoting tool, prepare a quote and email the quote to your client.
  • Have your client complete the online application and submit the first month's payment via credit card.

Submit to Kaiser Permanente via email
  1. Have your client complete the membership application form for Maryland, or Virginia.
  2. Include your client's first month's premium.
  3. Mail the materials directly to Kaiser Permanente at the address on the enrollment application. If you are working with a general agent, please submit the materials to your general agent.

Reminder: Make sure the application is complete, signed and includes the first month's premium payment. If the application is incomplete or does not include the first month's payment it may delay the enrollment effective date or the application may be canceled.

If you don’t have access to the SMU portal but want to enroll your client as soon as possible, go to www.buykp.org and guide them online to choose a plan, fill out an online application and include your information on the broker page.
